2. CPC兒童產品證書(Children’s Product Certificate)
CPC需要包含7個要素
a. Identification of the product covered by this certificate: 產品識別，可以寫產品名稱型號等
b. Citation to each CPSC product safety regulation to which this product is being certified:產品符合的美國法規或者標準，產品不同可能適用的法規標準會有差異，此處需要根據實際產品的測試報告填寫
c. Identification of the importer or domestic manufacturer certifying compliance of the product:美國進口商或者美國國內制造商
d. Contact information for the individual maintaining records of test results:測試結果維護人的聯系方式，一般寫測試申請商的聯系方式
e. Date and place where this product was manufactured:產品何時何地生產的，日期寫到月份即可；地址寫到城市即可
f. Date and place where this product was tested for compliance with the regulation(s) cited above:產品測試的時間和地點
g. Identify the third party, CPSC-accepted where this product was tested by an accredited laboratory (accepted by the CPSC) for compliance with the regulation(s) cited above:測試產品的實驗室及聯系方式，此處要求必須在CPSC認可的實驗室做測試
